To ensure you’re eligible for treatment, you must:

• Be registered with an NHS GP practice 
• Have a confirmed diagnosis in the treatment area you are seeking from your NHS or equivalent primary healthcare provider.
• Have evidence of two previously tried licensed treatments, which we will review as part of your assessment.

To verify your eligibility, we’ll need these details recorded in your GP records. With your consent, we will contact your GP practice to confirm this information.
